I haven't merged 401k plans, but we did switch providers from ICMA-RC/MissionSquare to Empower last year (in addition to URS plans). I was so grateful to have the help of retirement plan gurus through our broker, GBS Benefits. GBS Retire were certified experts and knew their stuff. We had issues on an RHS plan, and they knew what to do. They gave us great information, and let us make the decisions. If you haven't already, check with your broker. If your broker doesn't do that, let me know and I can check with GBS Retire.
